GVC Ultimate Driving Schools - Advanced Driver Program - Forms

Pre- Event Forms

These are mandatory forms that ALL driving school participants and instructors need to print, read, complete, sign and bring to each event. You will not be allowed to participate in the event without first presenting a completed and signed Technical Inspection form at Check-in.

The Technical Inspection Form specifies the minimum requirements for the safety of your track vehicle. We STRONGLY suggest that you have a competent professional mechanic inspect your car and make any needed repairs. The inspector should be familiar with demands placed on vehicles at HP driving schools. The inspector must sign the Tech form to verify the inspection results.

The GVC reserves the right to reject any vehicle that is deemed unsafe for track use.  No car failing inspection may be used on the track until it is repaired, re-inspected, and passes tech.

Emergency Medical Information and Contact Form:The medical /contact form must be filled out, sealed in a #10 business envelope, and submitted when you check-in at the event.

However, we now require participants to list on their Tech form the name of a responsible person to be contacted in an emergency, and a phone number where the contact can be reached during the event. Tech forms lacking this information will be considered incomplete, and the driver will not be permitted to participate in the event until such information is provided.
For anyone bringing children under 18 years old to the event - Both parents or legal guardians of minors under 18 years old are required to sign a Minor Waiver upon entering the event premises. If either parent or legal guardian is not present at the event, this waiver must be signed and notarized prior to the event by the parent(s) / guardian(s) not at the event and presented to GVC administration on arrival at the event.

Please note: The Minor Waiver form must be printed IN COLOR or it will not be accepted.

Students use this form as a guide to evaluate their instructor, classroom sessions, and overall event.
Instructors use this form as a guide to evaluate their students.
This document describes the expected level of driving skill for each run group.
